As part of the Tennessee Fire Mortality Research Project, the State Fire Marshal's Office is asking for assistance in tracking fire related deaths and injuries in Tennessee.

Fire departments, law enforcement, EMS and resuce agencies are asked to report deaths or fire-related injuries within 24 hours of the event.

The reporting form can be filled out on a computer using Adobe Reader (download free here).  The completed form can be printed and faxed to the State Fire Marshal's Office at 615-741-1583 or saved and emailed to This e-mail address is being protected from spambots.
